Early Intervention Service Coordination
The Early Intervention Program works with children under the age of 3, experiencing a developmental delay or who have a diagnosis that has a high probability of resulting in a developmental delay. Evaluations are provided at public expense with no cost to the family. If your child meets eligibility criteria for the Early Intervention Program, our office will assign a service coordinator to work with your family to assure access to both individualized early intervention services, as well as to other community programs and resources they may be eligible for. If you suspect your infant or toddler may be experiencing a developmental delay(s), seek help early. A referral can be made by calling the Regional System Point of Entry toll free number at 1-888-653-4463.
Case Management Program
The Special Child Health Services Case Management Program is committed to assisting families in caring for their children (age birth to 21) with complex, long-term medical and developmental disabilities. A case manager will be assigned to your family to assist you with coordinating comprehensive services (medical, social, economic, educational and recreational) for your special needs child.

To make a referral to the Early Intervention Program, please call the Regional System Point of Entry, toll free, at 1-888-653-4463. To make a referral to the case management program, please call our office between 7:30am and 5pm at 201-915-2514.
We also welcome referrals from physicians, hospitals, daycares, social workers, schools, and community agencies.
