Admitted with StrokeWhat happens after a patient is admitted to JCMC with a stroke diagnosis? Patients are admitted to the ICU, Stroke Unit or a Telemetry unit. Nurses trained in the care of the stroke patient will coordinate an individualized plan of care for each patient. Patients are evaluated for physical, occupational, speech and/or swallowing therapy. A diet is ordered based on the results of the patient’s swallow evaluation and dietary needs. Stroke education is provided to patients and family members which includes topics such as personal modifiable risk factors, stroke warning signs and symptoms and the importance of calling 911 for stroke-like symptoms. Referrals are made to our Diabetic Educator, Dietician, “I Quit Smoking” and Wealth from Health programs on an individual basis. A discharge plan is initiated with a referral to the appropriate level of care.
