Welcome to Patient Navigation Program Jersey City Medical Center Innovative, Reward-Based Program for Our Employees and Patients

Patients with chronic health conditions benefit from high-quality, coordinated outpatient care. This coordinated care includes patient-managed tasks and educational activities that are 'best practices'. Health outcomes from studies show improved health results including a better quality of life, fewer complications, less hospitalizations, and lower overall health care costs.

How does the Patient Navigation Program program work?

WHO: Patients or employees of Jersey City Medical Center who wish to best manage their chronic health conditions.

WHAT: The Patient Navigation Program is an innovative reward points program to empower you as a patient to effectively manage your chronic health condition as a true partner in your care. This is a free, voluntary program with tangible rewards to you. You will be assigned a Patient Navigator who will support you and provide you basic, effective, practical tools and education to ensure that you can take control of your health.

This program is for individuals to earn points for reaching self-management goals, which can be 'cashed' in for gift cards or products. Being part of the Patient Navigation Program club also gets you discounts at local vendors in your community.

WHEN: As a program participant you will receive a picture ID membership card which will allow you to get discounts with several vendors throughout Hudson County with expansion planned. From the moment you join who receive instant rewards!

WHERE: Our current vendors are located within Hudson County with an expanding vendor participating list.

For primary care, we offer access to JCMC's Inner Circle of physicians who are located throughout Hudson County and include our partnering Federally Qualified Health Centers

WHY: We all respond best to rewards that are immediate and can be held in our hands. Providing you an incentive to pick up your medicine, fill out a chart, or get your flu shot for example is a smart use of 'health care resources' and can help you live a healthier and happier life.

You earn reward points by:

  • Meeting with your dedicated Champion Navigator and gaining awareness about the cause, common symptoms and complications of your conditions.
  • Attending doctor appointments and taking your medications as prescribed.
  • Making personal goals about your health that you would like to accomplish.
  • And more….
