MMC and MMC, Southern Campus to Provide New Behavioral Health Offering


The Barnabas Behavioral Health Network will now participate in the New Jersey Delivery System Reform Incentive Payment program, by creating a new Behavioral Health offering ‐ The Integrated Health Home for the Severely Mentally Ill.

The Integrated Health Home is a wellness program designed to provide behavioral and physical health services for those with severe mental illness, ensuring that each client has an ongoing relationship with both a medical and a psychiatric licensed independent practioner using one treatment plan.

Beginning October 1, the program will serve Monmouth and Ocean counties—with locations at MMC's Pollak Clinic and the Barnabas Health Behavioral Health Center—and will include case management, client and care‐giver educational tools, staff and provider education, social support assessment, referral interventions, client self‐care plan, nutritional support, and home visits.
