Center for Healthy Aging at Monmouth Medical Center, Southern Campus Presents “Take Control of Your Health”


Those living with a chronic disease often struggle to control their symptoms. To help older adults and their caregivers manage the day-to-day issues associated with chronic disease, the Center for Healthy Aging at Monmouth Medical Center, Southern Campus, a Barnabas Health facility, will host "Take Control of Your Health" – a chronic disease self-management program developed by Stanford University – beginning January 12.

Over the course of six weeks, health coaches from the Center for Healthy Aging will teach participants about chronic illness, help them learn from one another about ways to cope with an illness, and identify ways they can live healthier by managing symptoms of chronic disease.

The free program is open to Ocean County residents their families and/or caregivers, and each workshop participant will receive a book on living healthy lives despite chronic disease. Training and materials for this program were provided by Community Services, Inc. of Ocean County. A light lunch will be served at the introductory session.

Sessions will be held every Monday, January 12 through February 23, from 11 a.m. to 1:30 p.m. at the Center for Healthy Living, located at 198 Prospect Street in Lakewood. Preregistration is required by calling 732-597-6075.
