The Case Management Department consists of Registered Nurses
and Social Workers who work closely with patients, families,
physicians and your health care team to ensure that the care
delivered meets each patients needs. Case Managers will facilitate
referrals for home care services, outpatient services, equipment
and transportation. Social Workers will assist with discharge
planning, short term counseling and referrals to appropriate
community agencies and post-acute facilities.
