Offering comprehensive evaluations for adults 65 and older, the Geriatric Health Center is staffed by board-certified geriatricians. A team of health care professionals works with patients and their primary care physician to provide a comprehensive assessment of health care and social needs, and to insure continuity of care with the referring physician.
Primary and consultative services are available for older adults suffering from:.
- Memory loss and confusion.
- Depression and behavioral changes.
- Decline in mobility, changes in gait and falls.
- Multiple concurrent medical conditions.
- Polypharmacy/Medication Management
The initial visit to the health center includes a complete physical examination, assessment of physical functioning and mental status, a review of all medication, laboratory testing and x-rays.
The health center's Memory Disorders Program provides a comprehensive evaluation of patients who exhibit signs or symptoms of dementia. Through careful analysis, the Geriatric Health Center's team of medical specialists uncovers the underlying condition of the disorder and recommends appropriate treatment.
