The Heart Centers offer individually customized rehabilitation with advanced techniques and state-of-the-art equipment for inpatients and outpatients. Nutritional counseling and psychological support services are part of every cardiac rehabilitation plan.

Recovering from and managing a cardiac condition is an intense process that requires patience, compassion and persistence.  Through innovative, leading-edge therapy programs and a commitment to following each patient through every step of his/her care, Barnabas Health has positioned itself as a leader in the field of post-acute cardiac care services.  Our specialized post-acute cardiac programs are developed in collaboration with the cardiac experts at the Barnabas Health Heart Centers ensuring that individuals have access to outstanding care and a variety of services when in need of additional support following a cardiac condition.  

Barnabas Health Home Health Care Services

Barnabas Health’s Home Health Care Cardiac Rehabilitation Program is a conditioning program which assists cardiac patients and families with the transition from the hospital setting to home. It enables patients to continue rehabilitation that was started in the hospital, in the comfort of their own home.  In conjunction with the patient’s cardiologist and primary care physician, the home health care physical and occupational therapists employ various activities to increase the patient's strength, reserve energy, relieve anxiety and enable him/her to maintain independence.  A registered nurse also visits each patient to conduct an initial assessment.  


Barnabas Health Hospice and Palliative Care

The Barnabas Health Hospice and Palliative Care Center offers an innovative Cardiac Special Care Program for people with advanced cardiac disease. When an individual has end-stage cardiac disease it affects the entire family.  With support that addresses both the illness and the unique needs of patients and families, everyone can experience a better quality of life.
