Barnabas
Health’s Home Health Care Cardiac
Rehabilitation Program is a conditioning program
which assists cardiac patients and families with
the transition from the hospital setting to home. It enables patients
to continue rehabilitation that was started in the hospital, in
the comfort of their own home. In
conjunction with the patient’s cardiologist and primary care physician,
the home health care physical and occupational
therapists employ various activities to increase the patient's strength,
reserve energy, relieve anxiety and enable him/her
to maintain independence. A
registered nurse also visits each patient to conduct
an initial assessment.

The Barnabas Health
Hospice and Palliative Care Center offers an innovative
Cardiac Special Care Program for people with advanced cardiac disease.
When an individual has end-stage cardiac disease it affects the entire
family. With support that addresses both the illness
and the unique needs of patients and families,
everyone can experience a better quality of life.
